What is the difference between activity, outcome, and impact?

Activity describes what was delivered; outcome observes a nearer change; impact examines a transformation and the intervention’s actual contribution.

Is a baseline always required?

A point of comparison proportional to the decision is required. It can be quantitative, qualitative, or mixed, but its limitations must be explicit.
